Required Documents For Patients

  • A copy of passport information page with certified signature
  • Updated all medical reports and relevant medical investigation results
  • Referral letter obtained from current hospital indicating the need for further treatment in Thailand 
  • Proof of financial status or sponsorship that covers treatment costs
  • Insurance coverage or guarantee letter from the embassy with a full coverage for COVID treatment in Thailand, medical evacuation and repatriation, if required. Minimal coverage is USD 100,000.
  • Letter of intention for treatment and consent form for hospital quarantine. These documents are prepared by Bangkok Hospital Headquarter and signed by the patient and /or attendant (s).
  • Fit-to-travel medical certificate issued by an authorized doctor in current country within 72 hours prior to arrival. The patient will be notified by Bangkok Hospital Headquarter when this document should be prepared.
  • Testing of COVID-19 (RT-PCR) with negative result issued within 72 hours prior to arrival. The patient will be notified by Bangkok Hospital Headquarter when this document should be prepared. COVID-19 test should not be obtained before an approval is assured.
  • Flight ticket or other transportation documents, if applicable.
  • Certificate of COVID-19 vaccination approved by authorized organization in the country of origin. Given COVID-19 vaccines must be approved by WHO or Thai FDA. Please see the list of approved vaccines

Required Documents For Attendants

  • A copy of passport information page with certified signature.
  • Affidavit of support (statement of relationship with the patient). This document is prepared by Bangkok Hospital Headquarter and signed by the patient and /or attendant (s).
  • Health insurance or travel insurance with a full coverage for COVID treatment in Thailand, medical evacuation and repatriation, if required. Minimal coverage is USD 100,000 per each attendant.
  • Fit-to-travel medical certificate for general passenger issued by an authorized doctor in current country within 72 hours prior to arrival. The attendants will be notified by Bangkok Hospital Headquarter when this document should be prepared.
