Processes to Ambulation Training

     A Purpose of rehabilitation training is to maximize patient’s ability up to level of ambulation or moving themselves. It is

expected that with appropriate care and  rehabilitative program, stroke and paraplegic patients, in particular, might be

able to perform their activity of daily living, transfer from place to place and ambulate with some supportive devices

with minimal assistant.  Processes to these functional abilities requires specific medical prescription  and training program

from physical therapists as follows:

 

     Step I: Muscle Strengthening program: Firstly, specific muscle weakness must be evaluated.  This depends

on underlying problem of the disease.  Testing of muscle power for individual is needed.  Particular attention is

Placed on those muscles involved with upright activity: back of neck and trunk, gluteal, quadriceps, and calf, In

some case, strengthening of upper arm: biceps, triceps is also concentrated.

        

        

 Step II: Standing balance Training: Balance Training on Bed: For some patient, who supines in bed for long period

of time, abnormal symtoms, dizzy, dropping of blood pressure might take place when he/she changes from supine to sitting

position.  These problems might be overcome gradually until vital signs are stable.  Exercise in bed, from lying flat to upright,

is a critical step to create physiologic adjustment.

         

Balancing with machine: With adjustable tilt table, patient is then trained to bear weight on knee and hip.  This machine

provides opportunity for patient to gradually adjust degree of inclination while vital signs: blood pressure, heart rate,

breathing frequency and oxygen saturation in the blood will be ,monitored as safety indicators.

                                                             

 Balancing within parallel bar: Within parallel bar, patient feels very comfortable and Self – adjustment body alignment is

possibly performed using a big mirror in front of patient.  This gives learning experience required during walking outside.

 

                                      

Step III. Walking outside the bar: To complete ambulation program, patient will be brought out of parallel

bar.  However, appropriate walking devices must be prescribed prior to this step.  Limb and back supports, as

well as walking stick, single cane and crutches will be considered for each individual in this step.

Techniques of ambulation: swing to, swing through, four-points gait are conducted.
