How to Request Medical Documents

Request in person or through a representative by filling out the medical document request form with supporting documents and emailing to [email protected] |
Required Documents for Medical Document Requests
1. If the patient collects documents in person:
– Copy of valid ID card/driver’s license/passport (for minors, use birth certificate/household registration showing the child’s name)
2. If someone else or a representative collects documents on behalf of the patient:
– Power of attorney stating the full name of the authorized person
– Copy of patient’s valid ID card/driver’s license/passport (for minors, use birth certificate and parent’s ID card)
– Copy of valid ID card/driver’s license/passport of the person collecting documents
Document Delivery/Collection Options
Customers can indicate their preference via BPH Connect or the request form through 3 channels:
1. Email
2. In-person collection
3. Postal delivery
Processing Time: 3-7 days (depending on document type)
