Package Information

• This vaccine package is for COVID-19 unvaccinated people only.
• This vaccine package can be service at Bangkok Hospital Chiang Mai only.
• The package contains 2 doses, vaccinated person for the 1^st and 2^nd shot must be the same person
• This COVID-19 vaccine is recommended for people aged 18 or older. For patients with certain underlying conditions or people who have particular concerns of vaccination, please consult the doctor prior to vaccination.
• Under any circumstances, personal detail for vaccination with Moderna COVID-19 vaccine cannot be changed or transferred to others
• This price includes vaccine (with insurance in case vaccine allergy develops), medical supply and hospital fees on the vaccination day only.
• This price NOT includes medical certification, other irrelated service fees, e.g., health checkup, doctor fee for other symptoms.
• This price is valid only for Thais and Domestic Expatriates
• The hospital reserves the rights to amend terms and conditions without prior notice.
• The hospital reserves the rights to issue no refund in all cases.

Vaccination Service Timeline

• Vaccine arrangements and distributions are proceeded by the Government Pharmaceutical Organization (GPO), Thailand, Bangkok Hospital Chiang Mai might not be able to commit the exact date of vaccine arrival
• At this stage, expected period of arrival of Moderna COVID-19 vaccine at Bangkok Hospital Chiang Mai is October to December 2021 (no specific date)
• Expected period of vaccination service at Bangkok Hospital Chiang Mai is October to March 2022 (no specific date )
• Your appointment for vaccination will be made by payment order. You will be further notified via Online Media once Moderna COVID-19 vaccine arrives at Bangkok Hospital Chiang Mai.
• In case that the Hospital gets Moderna COVID-19 vaccines from by the Government Pharmaceutical Organization (GPO) later than 31 December 2021, the hospital will schedule your appointment of vaccination during 1 January – 31 March 2022.

Other conditions

1. The vaccine quantity proceeded by the Government Pharmaceutical Organization (GPO) is limited, not adequate to fulfill all need of all customers.
2. According to physical distancing protocol, as your safety is our priority, the hospital manages to sell the vaccine package only via official online channel. However, online access from multi users simultaneously may cause errors or discomfort, the hospital, therefore, apologizes for any inconvenience this may cause
3. Thai government has free Covid-19 vaccination program, however for those who wish to get alternative vaccine through private hospital, you are responsible for your own cost.
