Health check-up for Australian visa

Preparation :

Required documents

1. Original passport of copy of passport.
2. For a baby do not have passport, may use the Citizen I.D. or birth certificate translate in English.
3. Document from embassy with a reference code (HAP ID, HRI, TRN).
4. Please bring medical records and medication lists on the visit date (if having any medical history) as our specialist needs to mention in the embassy application.
5. If the applicant age is less than 18 years old, should be accompanied by parent or guardian for signing consent.
6. COVID Vaccine certificate (print out from MorProm) to upload in the embassy application.

Health check preparation

1. Absolutes fasting is not required.
2. Should avoid coffee, tea and soft drink to reduce the risk of high sugar level in urine.
3. If wearing glasses or Contact Lenses, please bring with you on the visit date.
4. For female, should book appointment 5 days after menstruation cycle.
