Cash limit of the B BEYOND HEART is valued at 80,000 THB. for medical expenditures spent on: Medical treatment fees for outpatient and inpatient departments Heart screening packages (Preventive Heart & Lipid Check-up Package, Heart & Lungs CT Scan Package, Cardiac Arrhythmia Screening Package and CT Coronary Calcium Score Package) Walk-in rates applied. Genetic Heart Screening Package and Pharmacogenomic Test for Antiplatelet Drug (Clopidogrel), Lipid Lowering Agents (Statin group) and Anticoagulant (Warfarin) Walk-in rates applied. Online doctor consultation service.	Services are available at Bangkok Heart Hospital (Soi Soonvijai) only. Benefits are transferrable according to the hospital’s specified conditions. These benefits must be used as specified and cannot be used in combination with other discounts, promotions, Health Expo coupons, Chivawattana card and other packages. In case the actual usage does not exceed the given value 500 / 1,000 / 3,000 and 5,000 THB. The hospital reserves the right to refuse a refund or exchange.
Heart and Health Check-up Packages (Please select 1 package) Healthy Heart Package – 14,000 THB. Service is available at Cardiac Center, Special Cardiac Center and Cardiothoracic Surgery Center OR Superior Check-up Package (Age 30 – 40) – 13,000 THB. Service is available at Health Design Center. This package is only applicable to people aged over 15 without the use of wheelchairs or mobile stretchers.	Services are available at Bangkok Heart Hospital (Soi Soonvijai) only. Benefits are transferrable according to the hospital’s specified conditions. Please make an appointment at least 1-2 days before receiving services. FREE! Vaccination (Please select 1 vaccine) Quadrivalent Flu or Influenza Vaccine (1 shot) – 1,000 THB. (including outpatient doctor fee) for people aged over 15 OR Pneumococcal Vaccine (1 shot) – 4,400 THB. Discounts 30% on “emergency ambulance services” for accidents, injuries and emergencies 24/7. And discounts 30% on “referring ambulance services” to receive treatments at Bangkok Hospital Headquarters (Soi Soonvijai).	This is non-transferrable. Discounts are applied to ambulance fee, excluding doctor fees, medical equipment, nursing fees and other service fees. This service is only permitted within Thailand. Prices are set in correlation to distances.
50% Discounts on inpatient rooms during hospitalization Discounts are applied to inpatient rooms on wards, excluding delivery room, observation room and nursery. Discounts are not applied to inpatient meals, nursing fees, hospital fees and other service fees as specified by the hospital. Services are only available at Bangkok Hospital Headquarters (Soi Soonvijai).	This is non-transferrable.
25% Discounts on chronic medications for heart diseases, both outpatient and inpatient visits Discounts are applied to selected groups of medicines defined by the hospital Services are only available at Bangkok Heart Hospital (Soi Soonvijai).	This is non-transferrable.
15% Discounts on inpatient and outpatient medical expenses Discounts exclude doctor fees, special medicines, special medical equipment and medical supply, special laboratory tests, special packages and promotions as well as fixed price packages. Services are only available at Bangkok Hospital Headquarters (Soi Soonvijai).	This is non-transferrable.
10% Discounts on cancer screening and detection using PET-CT scan from package price Please make an appointment and present the member card prior to services. The right is reserved to the membership holder and it is not transferrable. Services are available at Bangkok Cancer Hospital Wattanosoth located in Headquarters (Soi Soonvijai).	This is non-transferrable.
5 – 10% Discounts on dental services 5% discounts on prosthetic dentistry (dental crown, dentures or dental bridge), esthetic dentistry and implant dentistry 10% discounts on tooth scaling, periodontics (gum diseases), pediatric dentistry, dental fillings, occlusion dentistry, root canal treatments and dental service fees. 5% Discount on special diagnostic equipment and machines Discount can be applied to X-Ray, ultrasound, CT scan and MRI scan. This excludes all package prices. Services are only available at Bangkok Hospital Headquarters (Soi Soonvijai).	This is non-transferrable.

This member card is reserved for Thai citizens and domestic expatriates residing in Thailand with valid work permit only.

- 24/7 ground-ambulance services for emergency, accident and hospital-to-hospital ground-ambulance services in the Bangkok area (designated to Bangkok Hospital Headquarters). Ambulance offer is up to 2 times/ year. (Please note, this offer includes ambulance service fee and excludes doctor fee, medical devices and supplies, medicines, nursing fees and other service fees.)

Membership terms and conditions

Please present your membership card prior to receiving services.
To transfer certain benefits to others, please present letter of attorney clearly stating name-surname of beneficiary and transferred privileges.
For corporate patients, given benefits and privileges cannot be used in combination with corporate payers unless applied to an out-of-pocket expenditure paid by the patients.
For patients who possess health insurance along with this member card, the patients may make payment using B BEYOND Heart Member Card first and bring the receipt from the hospital to proceed reimbursement from the insurance company. Or this member card can be applied later after the company’s Fax Claim service under the conditions specified by the hospital.
In case of losing card or re-changing names of members, please contact Chivawattana counter, Bangkok Hospital Headquarters to reissue the card. The fee is 100 THB.
If the issued card needs to be revoked for any reason, Bangkok Hospital Headquarters reserves the right for not giving any refunds.
All benefits, discounts and privileges are applicable as stated by the hospital.
