Terms and Conditions :

1. Members must present their Chivawattana card in order to redeem a discount.
2. Asterisked items (*) are transferable (transfers apply to immediate family members only); Junior Card benefits may only be transferred to immediate family members up to 15 years in age.
3. When a privilege is transferred, the recipient must provide the Chivawattana card of the transferor (or a copy), indicating the name of the recipient and the transferred item(s).
4. Per visit discounts cannot be combined for the same treatment, unless stated otherwise.
5. If for any reason any service specified is not available at one hospital, the privilege can be used at any other participating hospital within the Bangkok Hospital Group, unless the service is only available exclusively at a specific hospital.
6. Membership renewal must be made within 30 days after membership expiration date to be eligible for discount.
7. Members cannot use Chivawattana benefits for medical services covered (discounted or otherwise) by their employer's health policy with participating Bangkok Hospitals.
8. Members, however, are eligible to use Chivawattana cards in conjunction with an insurance policy that includes participating Bangkok Hospitals. The Chivawattana discount will apply first, and the member can then claim the balance from their insurance company.
9. In case of lost or damaged card, please inform your hospital for a new card. There will be a fee of 100 Baht.
10. The card cannot be transferred except in the case where the member is deceased. Only immediate family members have the right to the transfer, with a fee of 100 Baht.
11. If the member decides to cancel a card, there will be no refund.
12. The hospital reserves the right not to allow the use of any Chivawattana privileges with other promotional programs or employer benefits program; unless otherwise stated, Chivawattana discounts apply only to regular priced exams.
13. Chivawattana reserves the right to alter any privileges without prior notice.
