The Heart Failure Program
Heart failure, or congestive heart failure,
is a chronic illness that has reached epidemic proportions. While the prognosis
is generally poor for heart failure patients, proper management and strict
adherence to evidence-based treatment practices can not only improve the
patient’s quality of life, but increase longevity as well.
Inclusion criteria
- Heart failure or congestive heart failure.
- Pulmonary congestion or pulmonary edema.
- Ischemic CM, non-ischemic CM, or DCM.
- A combination of orthopnea, dyspnea and edema.
Program Procedure
Evidence-based practices are followed upon entering
the heart failure pathway:
- Preprint orders are utilized that follow evidence-based guidelines.
- Core practices (practice indicators) are completed.
- Multidisciplinary care is commenced.
- The patient and family are counseled and educated to assure proper
self-management.
- A smooth transition is made from inpatient to outpatient status.
- Core practices and outcome are tracked and measured.
- Care data is analyzed to continually improve practice methods.
