The Heart Failure Program

Heart failure, or congestive heart failure, is a chronic illness that has reached epidemic proportions. While the prognosis is generally poor for heart failure patients, proper management and strict adherence to evidence-based treatment practices can not only improve the patient’s quality of life, but increase longevity as well.

Inclusion criteria

  1. Heart failure or congestive heart failure.
  2. Pulmonary congestion or pulmonary edema.
  3. Ischemic CM, non-ischemic CM, or DCM.
  4. A combination of orthopnea, dyspnea and edema.

Program Procedure

Evidence-based practices are followed upon entering the heart failure pathway:

  1. Preprint orders are utilized that follow evidence-based guidelines.
  2. Core practices (practice indicators) are completed.
  3. Multidisciplinary care is commenced.
  4. The patient and family are counseled and educated to assure proper self-management.
  5. A smooth transition is made from inpatient to outpatient status.
  6. Core practices and outcome are tracked and measured.
  7. Care data is analyzed to continually improve practice methods.
