Certificate of Distinction : DM type 2 program

Diabetes is a chronic disease that needs continuous care. The goal is to have diabetic patients learn how to control their blood sugar levels to normal or near normal as possible to prevention chronic and acute complication. However, it is requires collaboration between the patients, family and the professional of healthcare team to achieve this goal. For this reason, the healthcare team has established a DM Type 2 Pathway in accordance with international standard to enhance the quality of life for diabetic patients.

Criteria for selection of patients to enter the program

  1. Inclusion
    • DM Type 2 diagnosis
    • Age 15 – 80 years old
    • HbA1C ≥ 7 % who visit OPD
  2. Exclusion
    • Do not meet classification DM type 2 by ADA
    • Cancer diagnosis, Classified Stage 4
    • Kidney failure on dialysis
    • Patient refusal
    • Bed ridden

Guidelines of treatment (Practice Guideline)

First visit patient:

  • Physician visit for treatment
  • Consultation with Diabetic coordinator
  • Consultation with Dietitian
  • Consultation with clinical pharmacist

Follow up patient at OPD:

  • Physician visit for treatment
  • Visit Diabetic coordinator when the HbA1C ≥ 7%
  • Visit Dietitian if the patient does not pass the evaluation of diabetic’s standard.
  • Visit clinical pharmacists if the Physicians adjust, add, or the patient has a medicine problem.

Type 2 DM who admit to hospital /ICU to observe critical Hypoglycemia or critical Hyperglycemia.

  • Physician visit for treatment
  • Blood tests according to protocols
  • Stop all DM Medication and follow protocol as ordered by physician.
  • DTx-according to protocols and adjust the date of insulin according to incaution scale, Follow Hypoglycemia protocols.
  • Consult Diabetic Coordinator, Nutrition, and Clinical pharmacist.
